The average time to complete this application is 20 minutes. This may take longer, depending on your household's situation.

When completing the Review and Modification Questionnaire, all fields marked with an asterisk [*] are required fields. You must provide this information in order to continue with the modification process. All other fields are not required for your modification application, but will help the Review and Modification Unit provide the most effective outcomes for your review and modification request.

Please have the following items available to assist you as you complete the request for review and modification:

• Social Security Numbers and dates of births for you and your children
• A copy of your original court order and all modifications for child support
• Health insurance policy information
• Employment/Self-Employment information
• Income information from all sources
• Income Tax Return Information/W-2's/1099's
• Copy of other court order(s) where you are ordered to pay child support

If you are uncertain about how to answer a question or you are experiencing any technical issues with the online review and modification questionnaire and/or website, please contact the Nebraska Child Support Customer Service Call Center at 1-877-631-9973, option 2. The Nebraska Child Support Customer Service Call Center hours of operation are 7:00 A.M. to 6:00 P.M. Central Standard Time Monday through Friday except government holidays.

To request a Review and Modification, you must have an open child support case. If it is determined you do not have an open child support case, you will complete an application for Child Support Enforcement Services by answering additional questions.

Once you have submitted your application, a PDF document of your application for Child Support Enforcement Services will be displayed. This is your verification that your application has been submitted and you should print or save the PDF document for your records. In addition, a PDF document of your request for Review and Modification will be displayed.
