- Establishing Court Orders for Child Support
- Establishing Court Orders for Medical Support
- Enforcing Order for Child, Spousal and Medical Support
- Modifying Child Support Orders (Upon Request)

The average time to complete this application is 20 minutes. This may take longer, depending on your household's situation.
When completing the application for Child Support Enforcement Services, all fields marked with an asterisk [*] are required fields. You must provide this information in order to continue with the application process. All other fields are not required for your application, but will help the Child Support Enforcement staff provide the most effective establishment and enforcement outcomes for your child support needs.
We cannot accept an application for child support services for an unborn child. You will need to wait until the child is born before you submit an application for child support services for an unborn child.
Please have the following items available to assist you as you complete the child support application:
- Social Security Numbers and dates of births for you and your children
- A copy of your court order for child support (if a court order has been established)
- Your child(ren's) health insurance policy information

Once you have submitted your application, a PDF document of your application for Child Support Enforcement Services will be displayed. This is verification that your application has been submitted and you should print or save the PDF document for your records.
